I have been astonished to discover my new GP insists on addressing me by my first name, which is Mary. And it's quite clear he does not expect his patients to call him "Bill". I find this very paternalistic. Any suggestions?

Unfortunately, one does occasionally find this attitude among older doctors. But for years now, it has been accepted patients are entitled to be addressed by whatever name they are comfortable with. So write to this guy's practice manager, explaining you want your GP to address you by your surname. If that doesn't work, change your doc.
