Health minister Hazel Blears has given the go-ahead for a further shake-up of services in Sussex.
The new Crawley Primary Care Trust will start operating on April 1.
It brings together GPs and more than 100 practice and district nurses across the area to make sure people get the health service they need.
It will also take over some services provided by West Sussex Health Authority, which is being dissolved.
This includes improving people's health through campaigns and awareness days.
Reverend Malcolm Liles, rector of the Crawley Team Ministry since 1998, has been appointed chairman of the trust.
Lynne Regent, director of performance development at East Surrey Health Authority, has been appointed chief executive.
