With reference to Dr Prashen Pillay’s error in prescribing an incorrect level of medication, surely other people must be equally to blame (The Argus, September 12)?
If his prescription of 500 milligrams had been given in full, it would have meant the tragic patient in this case being given hundreds of doses of medication.
Medical staff should have realised there was an error and queried the prescription.
