An inquest into the death of a man who was found hanged in a mental health hospital continued yesterday.

Oliver Minkley, 35, was discovered by staff in the men’s toilets of Mill View Hospital in Hove on May 2 - less than two days after he was admitted to the hospital.

Yesterday the inquest at Hove Crown Court was told that Mr Minkley, of The Drive, Hove, was put on general observation at Mill View – meaning he should be checked on once an hour by staff.

The inquest was told forms that should have been filled in when Mr Minkley was admitted were only partially completed.

The admission checklist that should have contained Mr Minkley’s details and the date he was admitted was left completely blank.

Senior staff nurse Patricia Nolan, who was on duty the night Mr Minkley was brought to Mill View, said admission protocol has been changed since his death.

On Monday the inquest was told Mr Minkley, who had a history of depression and anxiety, was brought by ambulance to the Royal Sussex County Hospital after his mental health deteriorated.

He arrived with his pregnant wife Gabrielle at about 6.30am on April 30 where he was initially assessed.

However he was not seen by the Crisis Resolution Team until about 3pm that day despite being referred four hours earlier.

Paul Beynon, service manager for urgent care, said the delay was partially due to an “unusually high amount of referrals” that day.

The inquest will continue tomorrow.
