More than a quarter of children in Brighton and Hove have had a filling before they go to school.
Health officials in the city say not enough children are being encouraged to brush their teeth properly.
Healthcare firm Simplyhealth's annual dental survey found that 28% of children in Brighton and Hove have their first filling before they are five.
The figure is the highest in the country and compares poorly to a national average of 11%.
